We understand the importance of your child’s health and wellbeing. That’s why we are committed to providing 15 community services in Wiltshire that cater to children, young people, and their parents and caregivers. We recognise that every child is unique and has different needs, which is why we offer a range of services that cater to their physical, medical, and mental health.
Since April 2016, we have been serving the Wiltshire community by bringing together services from five providers to create a focused children’s services team. Our services include public health nursing (also known as health visiting and school nursing) and specialized support for children and young people with physical, sensory, or learning disabilities.
Bath and North East Somerset, Swindon and Wiltshire (BSW) Integrated Care Board commissioned us to combine the services from five providers into a single entity. The goal was to increase the voice of children and families in the service and provide a joined-up service where families only need to explain their situation once. All families, regardless of their location, should have access to the same services.
We provide our services in the community, families’ homes, schools, and children’s centers, and we aim to see patients as soon as possible. In most cases, you will need to be referred to us by your GP, midwife, or another health or education professional. Our skilled teams are dedicated to delivering excellent care, and we focus on their development, empowering them to make a difference. We prioritize the safety of every child we care for and have comprehensive governance and reporting systems in place to monitor the performance of our services.
‘We Care’, ‘We Think’ and ‘We Do’: We care about the person in front of us, we think about how to make things better for them and we do something about it.
We’re proud to have made a positive impact up and down the country by designing and delivering services that address the wider economic, social, and environmental challenges faced by the communities we serve.
Health and care commissioners across the country trust us to work with them and their communities to transform services with a focus on experience, efficiency and improved outcomes. We deliver and transform adult and children community health services, primary care services including urgent care and sexual health as well as child and adult social care and wellbeing services.
Across England, we support communities of many millions and directly help more than half a million people each year.
